Write UNKNOWN if not available; however it is imperative for us to have it, this is how we know from which classroom to pick up your child. It is YOUR RESPONSIBILITY to get back to us via email with the necessary information.

Release of Liability: I am enrolling my child to participate in Hip Hop Hooray’s dance program. He/she is in good health, and I know of no physical or mental defects that would endanger his/her own well-being or that of any other student. I hereby release liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my child’s participation in Hip Hop Hooray, Inc. activities, and do hereby release and forever discharge Hip Hop Hooray, Inc., their affiliates, independent contractors, managers, members, agents, staff, volunteers, heirs, representatives, predecessors, successors, assigns, PPCS and all of their campuses, CPP, SBBC and any of their schools, venues and/or rented premises and any of their staff for any physical or psychological injury, including but not limited to any kind of damages, economical or emotional loss, that I, any predecessors, any successors or the child enrolled may suffer as a direct result of participation in the aforementioned activity, including traveling to and from an event related to this activity. I understand that my child is expected to behave in a manner that is not detrimental to any other student’s health or learning. If behavioral issues become a problem, I will be notified, and if they persist, I will be asked to withdraw my child from the program. If any pandemic arises, we must follow the Center for Disease Control guidelines. There will be NO makeup classes for any NO SCHOOL or EARLY RELEASE days as they have been considered when the monthly tuition fees were determined. Accounts must be paid in full by 4/20 (4/30 if it is an extended session) for the student to participate in any of our end of the year performances. No refunds/make-ups due to reasons including but not limited to, acts of God, riots, wars, insurrection of military power, civil rebellion, strikes, hurricanes, tropical storms, earthquakes, floods, natural disasters, or inclement weather (regardless of severity) or extraordinary school activities, CPP or SBBC events at any leasing facility. Personal absences are forfeited. It is the parent’s responsibility to keep records up to date, i.e. Who to dismiss your child to, teacher’s name, phone numbers, and/or emails address but not limited to.

FEES PAYMENTS ARE DUE THE 20th OF EACH MONTH FOR THE FOLLOWING MONTH OF CLASSES. Payments will be marked late by the 28th of the month if not yet received by the 28th a $15 late fee will be added to your invoice. All invoices MUST by the end of April to ensure eligibility in our End-of-the-Year Showcase. All Schools currently have, but not limited to 9 monthly installments of $63 based on a prorated tuition of 30 sessions annually per child. (Cost is prorated over 9 months for 30 sessions *see note for more information*) Lesson/session defines as follows: Every 45 minutes we are together as a group for a lesson, session, association, showcase, presentation and/or rehearsals. *Price per lesson/session is $18.90. We invoice the total 30 sessions (unless it’s an extended school year) All early release days, holidays, no school days, and/or day Hip Hop Hooray is closed has been taken into consideration and are NOT included in the 30 sessions. Monthly tuition does not decrease or increase regardless of the number of weeks during the month as the tuition is based on an annualized program.In a school year should the lessons/sessions need to be extended for any number of sessions a 10th payment due by the end of April will be required or the prorated amount will be added to the 9th payment. Accounts must be paid in full by the end of April. * $18.90/session x 30 sessions = $567 $567/9 months = $63 monthly Why do we do it this way? It is easier for everyone to remember and budget for the monthly installment of $63. If at any moment, or for whatever reason your child withdraws from the program, we will adjust the invoice for whatever number of sessions they were enrolled in. If you have credit we will issue a check, if you owe, we will bill you. Either must be taken care of within 14 days. PLEASE notify us two weeks prior to the withdrawal date as all the parties involved need to be notified. Dance outfits are required for those students participating in our annual showcase and/or any other presentation to ensure uniformity. Such outfits (top and bottom) should not exceed a $125 non-refundable fee (this excludes shoes, socks, undergarments, hair, and make-up) Tickets for our showcase can range from $21 -$35 (all sales are final). As our showcase date approaches (May) emails will be sent with all the necessary information. Numbers To Remember: 45 minutes session: $18.90 Registration: $30 per school year. – Non-refundable. Media Release: As a participant and/or parent/legal guardian of each respective participant enrolled in any activity with Hip Hop Hooray, Inc., I understand that myself and/or biological, ward, adoptive or foster child’s picture may appear in newspapers, websites, school publications, class pictures, or other communication tools. I further understand that Hip Hop Hooray, Inc. may use these photos and/or videos on electronic or in print advertisements or publication without my compensation at any given time and regardless of the year my child participated in the program. Use of Photo/Likeness; Consent: In consideration of the rights granted to (Hip Hop Hooray-HHH) by the (City of Pembroke Pines – CPP ) to lease their premises, the CPP may take publish, post, and make use of any all photos or video recordings taken during engagement and may use such media for promotional purposes, including possible use by the CPP flyers, brochures, advertisements, online, for social media, and in other prints and digital communications without payment or any other compensation to anyone involved.
