Write UNKNOWN if not available; however it is imperative for us to have it, this is how we know from which classroom to pick up your child. It is YOUR RESPONSIBILITY to get back to us via email with the necessary information.

Release of Liability: I am enrolling my child to participate in Hip Hop Hooray’s dance program. He/she is in good health, and I know of no physical or mental defects that would endanger his/her own well-being or that of any other student. I hereby release liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my child’s participation in Hip Hop Hooray, Inc. activities, and do hereby release and forever discharge Hip Hop Hooray, Inc., their affiliates, independent contractors, managers, members, agents, staff, volunteers, heirs, representatives, predecessors, successors, assigns, PPCS and all of their campuses, CPP, SBBC and any of their schools, venues and/or rented premises and any of their staff for any physical or psychological injury, including but not limited to any kind of damages, economical or emotional loss, that I, any predecessors, any successors or the child enrolled may suffer as a direct result of participation in the aforementioned activity, including traveling to and from an event related to this activity. I understand that my child is expected to behave in a manner that is not detrimental to any other student’s health or learning. If behavioral issues become a problem, I will be notified, and if they persist, I will be asked to withdraw my child from the program. If any pandemic arises, we must follow the Center for Disease Control guidelines. There will be NO makeup classes for any NO SCHOOL or EARLY RELEASE days as they have been considered when the monthly tuition fees were determined. Accounts must be paid in full by 4/24 (4/30 if it is an extended session) for the student to participate in any of our end of the year performances. No refunds/make-ups due to reasons including but not limited to, acts of God, riots, wars, insurrection of military power, civil rebellion, strikes, hurricanes, tropical storms, earthquakes, floods, natural disasters, or inclement weather (regardless of severity) or extraordinary school activities, CPP or SBBC events at any leasing facility. Personal absences are forfeited. It is the parent’s responsibility to keep records up to date, i.e. Who to dismiss your child to, teacher’s name, phone numbers, and/or emails address but not limited to.

Pricing and Payment Information: Registration Fee: $30 per child/per school year. Non-refundable. Cost: $69 every four weeks / $20.90 per lesson/session. Late Fee: $15 fee is applied if payment is not received within a seven-day grace period. Due Dates: 2024-2025: 8/15/24, 9/12/24,10/10/24,11/07/24,12/05/24,1/02/25, 1/30/25, 2/27/25, 3/27/25, 4/24/25. Installments: There are 10 installments of $69, based on annual tuition for 33 sessions unless it is an extended school year. Each payment remains the same, regardless of the number of weeks in the month. Extended Sessions: If additional sessions occur during the school year, a prorated fee will be charged and must be paid by the last payment date. Exclusions: The 33 sessions do not include early release days, holidays, no school days, or our "Hip Hop Hooray is Closed" dates. Payment Deadline: All accounts must be paid for in full by the end of April to participate in the end-of-year showcase. Sessions: Sessions are defined as follows: A session is every 45-minute period where we are together, this includes but is not limited to group activities, class, showcases, presentations, and rehearsals. Withdrawal Information: Withdrawal: If your child withdraws from the program, we will adjust your invoice based on the number of sessions they were enrolled in. If you have credit, we will issue a check. If you owe a balance, we will bill you. Please settle any balance within 14 days. Notification: Please notify us two weeks before the withdrawal date to ensure all parties are informed.  Costume and Tickets Information: Costumes: Students participating in our annual showcase and other presentations must wear specific dance outfits. These outfits should not exceed a non-refundable fee of $125 and do not include props, shoes, socks, undergarments, hair, or makeup. Tickets: Tickets for our showcase range from $21 to $35, and all sales are final. The showcase is in May. We will send all necessary information via email as the date approaches.  Disclaimer: We reserve all rights. Media Release: As a participant and/or parent/legal guardian of each respective participant enrolled in any activity with Hip Hop Hooray, Inc., I understand that myself and/or biological, ward, adoptive or foster child’s picture may appear in newspapers, websites, school publications, class pictures, or other communication tools. I further understand that Hip Hop Hooray, Inc. may use these photos and/or videos on electronic or in print advertisements or publication without my compensation at any given time and regardless of the year my child participated in the program. Use of Photo/Likeness; Consent: In consideration of the rights granted to (Hip Hop Hooray-HHH) by the (City of Pembroke Pines – CPP ) to lease their premises, the CPP may take publish, post, and make use of any all photos or video recordings taken during engagement and may use such media for promotional purposes, including possible use by the CPP flyers, brochures, advertisements, online, for social media, and in other prints and digital communications without payment or any other compensation to anyone involved.
