Please select the size your is expected to fit by May. Each outfit is sold as a single set-not multiple separate pieces. REFER to the size chart you received to ensure the selected size is available; some sizes may be crossed out if unavailable.

Please review each acknowledgment carefully. Submission of this form constitutes acceptance of all terms and policies.

Sizing Verification & Final Sale Agreement.(Required)

I certify that I have reviewed and confirmed my child’s size, school, and grade information and have selected sizes based on the provided size chart. I understand that sizing may run slightly small. I acknowledge that all outfit purchases are final sale, non-refundable, and non-returnable.

Performance Package Fee & Inclusions(Required)

I understand that the performance package fee has not yet been finalized; however, the total cost will not exceed $175. I acknowledge that this fee is non-refundable and includes one (1) dance outfit, two (2) show tickets, and one (1) refillable straw water bottle. I understand that the package does not include props, shoes, socks, undergarments, hair styling, makeup, or additional accessories unless stated otherwise in writing.

Terms, Responsibilities & Assumption of Risk(Required)

I have read, understand, and accept all terms and responsibilities. I acknowledge that the organization is not responsible for any lost, stolen, misplaced, or damaged outfits, costumes, props, or personal items at any time.

Exchange Policy Agreement(Required)

I understand and agree to the exchange policy. Exchanges are permitted only for unworn items with original tags attached and intact packaging, within the designated exchange period. A $20 exchange fee applies for shipping. Additional shipping charges may apply for expedited or air shipping. Delivery timelines cannot be guaranteed due to variable shipping factors.
