1. Assumption of Risk & Waiver of Liability
I acknowledge that dance and physical activity involve inherent risks, including but not limited to slips, falls, collisions, and injuries. I voluntarily assume all such risks on behalf of my child.
I certify that my child is in good health and free of any condition that would pose a risk to themselves or others.
I, on behalf of myself, my child, our heirs, representatives, successors, and assigns, hereby waive, release, and discharge any and all claims or causes of action against Hip Hop Hooray, Inc., its affiliates, managers, members, instructors, volunteers, contractors, agents, successors, PPCS, all PPCS campuses, SBBC, CPP (City of Pembroke Pines), any leased school or venue facility, and their staff, for any physical or psychological injury, emotional distress, economic or other loss that may result from my child’s participation in any activity, including travel to and from events.
2. Behavior Policy
I understand that my child is expected to behave in a respectful and cooperative manner. If behavioral concerns arise, I will be notified. If such behavior persists, I may be asked to withdraw my child from the program.
3. Health Emergencies
In the event of an emergency, I authorize Hip Hop Hooray, Inc. and its staff to obtain emergency medical care for my child. I release them from any liability related to such care.
4. Public Health Compliance
If any pandemic or health crisis arises, I agree to comply with CDC and public health guidelines.
5. Tuition, Refunds & Class Interruptions
There are no make-up classes for no-school days, early release days, or personal absences, as these are considered when determining monthly tuition.
Accounts must be paid in full by April 24 (or April 30 if part of an extended session) for participation in the year-end performance.
No refunds or credits will be given due to: Personal absences, Natural disasters (hurricanes, floods, severe weather alerts, etc. Acts of God, war, strikes, riots, or civil disturbances or extraordinary school activities, closures, or events related to CPP or SBBC
6. Media Release & Consent
I understand and agree that my child’s photograph, video, or likeness may appear in newspapers, websites, school publications, social media, advertisements, and/or promotional materials, both electronic and printed. I give permission for Hip Hop Hooray, Inc., the City of Pembroke Pines (CPP), and/or their affiliates to record, publish, and distribute these materials without compensation, now or in the future, regardless of the year of participation.
I release all rights to these images and understand that they may be used in brochures, flyers, videos, and online platforms.
7. Parent Responsibilities
I am responsible for keeping my contact information, dismissal list, teacher name, and other relevant records accurate and up to date at all times.
8. Policy Changes
I understand that all policies, terms, and conditions are subject to change based on the school year calendar or changes made by leasing agents, schools, or city partners.
9. Right to Refuse or Terminate Enrollment
We reserve the right, at our sole discretion, to accept, deny, or terminate enrollment of any participant or family at any time, with or without cause. Grounds for denial or termination may include, but are not limited to, a history of disruptive, threatening, harassing, or otherwise inappropriate behavior by the participant or their parent(s)/guardian(s), including but not limited to failure to comply with program policies, verbal or written threats, harassment of staff, defamation, or any conduct that interferes with the safe and respectful operation of the program. Our decision in such matters shall be final and not subject to appeal.
10. Governing Law & Severability
This agreement shall be governed by the laws of the State of Florida. If any portion is deemed unenforceable, the remaining terms shall remain in full force and effect.
